What facet and osteophyte findings mean, how uncertain facet-pain diagnosis is, and how treatment decisions are made.
Facet arthropathy and osteophytes are common age-related findings. They may be clinically relevant, contribute to stenosis, or be incidental. Pain with extension or rotation, local tenderness, or buttock referral can raise suspicion but does not confirm facet pain or reliably distinguish it from other causes.
